The number of reported AI-related incidents has increased. However, confidence in managing these incidents has decreased among organizations. The share of organizations reporting AI incidents remained steady at 8% in both 2024 and 2025. Yet, among those reporting incidents, the group experiencing 3–5 incidents grew from 30% in 2024 to 50% in 2025. Conversely, organizations reporting only 1–2 incidents dropped from 42% in 2024 to 29% in 2025. This shows a shift towards more frequent occurrences of multiple incidents.

Organizations also rate their incident response capabilities lower. In 2024, 28% rated their response as “excellent,” which fell to 18% in 2025. Those rating their response as “good” also dropped from 39% to 24%. The share describing their response as “satisfactory” increased from 19% to 32%.
